  Multiple claims can be submitted by entering your claim information on an excel sheet in the given format (click here for a sample spreadsheet) and then e-mailed to us. (Note: Save the sample spreadsheet, do not simply open it. Saving this to your desktop will allow for faster claim preparation in the future.)
• Submit individual claims via fax or postal mail
